DIALOG+ is a simple questionnaire which asks service users/patients about various aspects of life that are known to have an impact on their mental and physical wellbeing and recovery.

This simple, yet effective tool is helping to drive impactful conversations between service users/patients and their healthcare providers.

All community mental health teams are now using this tool to help them learn more about an individual’s family, relationships, leisure activities, accommodation as well as their physical and mental health.

Ogechukwu Ugwudike, Support Time Recover Worker based at the Lyndon Clinic shares how beneficial she finds DIALOG+. Working with 18–25-year-olds, Ogechukwu explains how DIALOG+ makes it easier to collaborate with her service users to identify their needs. She also shares how DIALOG+ enables her to track and monitor the mental health progress of her service users, noting significant improvements or alerting to rapid decline.
