Gold: North Hub Community Mental Health team
The North Hub Community Mental Health team aimed to transform the management of prescription forms (FP10s) requiring Essential Share Care Agreements, employing improvement techniques to enhance efficiency and service quality.
Through meticulous data analysis and targeted interventions, the team achieved remarkable results, positively impacting both staff and service users.
As a result of their work, there were many good outcomes including a 27% reduction time spent by medical secretaries and reception colleagues on handling FP10-related calls.
Despite facing significant barriers such as staffing issues, the team persevered and delivered outstanding results.
Silver: Dr Katarina Lietavova
The Sutton Home Treatment team undertook a quality improvement project to tackle the issue of reducing DNAs (Did Not Attend). By the end of the project DNAs reduced by 50%.
The project, undertaken by Dr Lietavova focused on her own patients. However, she worked hard to share what she learnt with other teams, in an effort to both inspire them and to demonstrate how to make improvements. Her passion for quality improvement and sharing all she has learnt has really shone through.
Bronze: Steps 2 Recovery Service
Our Steps 2 Recovery team embarked on a transformative journey compelled by the need to comprehensively evaluate their service. In response to feedback from the CQC and peer reviews, their determination was ignited to initiate change and cultivate a culture of continuous improvement.
Uniting Dan Mooney House, David Bromley House, Endeavour Court, Forward House, Grove Avenue, Hertford House and Rookery Gardens, the team were determined to identify what mattered the most to them and their service users. All teams enthusiastically embraced the challenge to elevate care provided, gaining necessary skills and capabilities along the way to drive change.
In addition to regular project meetings, all areas convened for quarterly ‘Share and Learn’ events, fostering a collaborative space to exchange successes, lessons learned, and challenges.
