A Care and Treatment Review (CTR), makes sure adults get the right care and treatment, it also looks at how to make care and treatment better for adults, there is also a Care (Education and Treatment Reviews (C(E)TR) which is different to a CTR because it is for children and young people.
The care and treatment review are for people who are at risk going into hospital or who are already in hospital, which includes children, young people, adults with a learning disability or autism. At BSMHFT we work with other departments within our trust to identify people who are at risk of going into mental health hospital and people who are already in a mental health hospital, we want to ensure you, and your family/carers/guardian are listened to and to have that full person centred input and therapeutic alliance with carers, to ensure a plan is made to support you and to get the treatment needed.
Each time a Care and Treatment review takes place, a review of red, amber, or green rating on the Dynamic Support Register takes place, both the Dynamic support register and Care and Treatment review can work hand in hand, a record of the CTR recommendations are kept on the DSR record. The review should include recommendation for people at risk of readmission.
As a trust we want to get it right, and ensure right support, right intervention, are done at the right time, a person who has learning disability and or autism in a hospital setting can be daunting, overwhelming, and stressful experience for an individual, with needs not understood and often overlooked as mental health. Please see a video by NHS England of an expert by experience Di Lofthouse on why she gets involved in Care and Treatment Reviews, and why it is important to put the individual and their feelings first. There is also an example on what a Care and Treatment planner looks (NHSE).
