The Dynamic Support Register (DSR) is a list of people with a learning disability and autistic children, young people or adults who need support because they’re at risk of going into hospital if they do not get the right care and treatment in the community, its vital that the right support is given as quickly as possible.

Most people with mental health difficulties or challenging behavior can live well at home, in their community and may never need to go into a specialist hospital for assessment or treatment. (Local Government Association)

Here at Birmingham and Solihull Mental Health Foundation Trust, we want to ensure the Dynamic Support Register works quickly as possible for you, as a service it is important to work together with other key agencies to ensure the service works for you, to find out what extra help and support, you need to be safe at home.

BSMHFT is keen to ensure a person-centred approach is adopted, you will be involved in when competing the referral form, to go on the register, its good to complete the form with someone who knows you well. Some of the questions will be about where you live and if you live with anyone, things that make you happy or upset, any other plans that may be in place, such as education, care, or health plans, and how best to keep you, and family safe.
