Enhancing Care Safety
To improve care safety in the NHS, all providers have adopted the new Patient Safety Incident Response Framework (PSIRF).
PSIRF is part of the national Patient Safety Strategy and changes how the NHS handles and learns from patient incidents.
We made necessary changes throughout 2023, and with approval from our local Integrated Care Boards, we went live with PSIRF in November 2023.
Learning from Incidents
We understand how much incidents affect patients, their families, carers, and staff.
It’s important to involve and support patients and families in how we respond to and learn from incidents to improve care.
PSIRF does not change our commitment to the Duty of Candour requirements.
A Just and Learning Culture
PSIRF is part of our commitment to building a fair and open culture. This means being transparent, treating everyone fairly, and learning from our mistakes.
PSIRF has four main goals:
– Compassionate engagement with those affected by patient safety incidents
– Using different methods to learn from these incidents
– Responding appropriately to each incident
– Strengthening our learning and improvements through supportive oversight
We will focus our reviews and investigations on areas where we can make the most significant improvements.

Our Approach and Response Plan
We are excited to introduce our Approach for implementing PSIRF and our Incident Response Plan for the coming year. Both documents will be regularly reviewed and updated as necessary. Our plan outlines the local safety priorities, identified in collaboration with key stakeholders, that we will focus on for the next 18 months.
